Covid-19 Community Relief Fund Application

Before you complete an application:

It is recommended that you complete the application in a word document prior to completing the online application. You can then copy and paste your answers into the application form.

You will be asked for the following information:

  1. Name of organization and primary contact information.
  2. Description - Please describe the organization. 
  3. Purpose of Funding - Describe specifically how the requested funds will be used.
  4. Impact - What impact will this have on the community?
  5. Support - What other community and financial support (funding, partnerships, in-kind, volunteers) have you been able to attract for this activity/service?
  6. Participants - Who is your target audience and how many clients or patrons do you expect to serve this year?
  7. Group/Committee/Boards Members

We will ask for the following documents to be uploaded during the application process:

  1. Completed Budget Form (Copy available in documents tab)
  2. Completed Signature Page (Copy available in documents tab)
